Evaluation forms for the MTF II 2002 Presentations are now available. Please download the evaluation forms for the various presentations. 

The final evaluation form (see below) is a two page sheet which should be filled at the end of the course. You can refer to the evaluation forms of the earlier presentations to fill out this form.

These Evaluation Forms are for the individual to use to record their comments after each session. Please submit only one evaluation form per presentation at the end of seven weeks.
